COVID-19 update Vaccination info 11 March 2021
This communication is for MACS resident’s listed medical next of kin.
In preparation of MACS being advised that we are scheduled to receive the COVID-19 vaccines we want to commence the collection of consent forms to facilitate the process. We have recently heard that some aged care providers are only receiving 48 hours’ notice of the vaccination team coming on site.
You may download the consent form here that requires your signature alongside some general information released by the Department of Health on the vaccine and some information on the Pfizer vaccine. We have arranged that we will have a dedicated staff member Serena available next Wednesday 17 March and Friday 19 March to meet with you at the Visitor’s Pod from 9.00 am to 12 noon & 12.30 pm to 2.00 pm to answer any questions you may have and or collect your signed consent form. Serena will also have consent forms for you to collect or to sign on the spot and return.
Should you have any medical questions in relation to the vaccine for your next of kin, we strongly encourage that you contact the Doctor caring for your next of kin. If you need assistance to facilitate this connection please speak to the either Kylie or Timea the Nurse Unit Care Co-ordinators.

As soon as we are made aware of when we may be receiving the vaccinations we will inform you. Plans are already in place to facilitate the administering of the vaccines, and monitoring afterwards. Extra staff will be rostered on to facilitate this.
